A new study by researchers of Boston University’s School of Public Health and colleagues sponsored by the World Health Organization (WHO) shows children with severe pneumonia can be effectively treated at home and do not need to be hospitalized. This finding is hugely significant for developing countries where children cannot be brought to a hospital easily or where no hospitals exist.

Per the study the change of treatment could save many children’s lives and take pressure off health systems. Every year pneumonia kills 2 million children under the age of 5. The researchers found that antibiotics given at home could significantly reduce deaths.

The group examined 2,037 children between 3 to 59 months in seven areas in Pakistan. About half of them were given antibiotics and sent home while the other ones got intravenous antibiotics in the hospital. Both groups were found to show equal progress in healing off the illness.

Current WHO guidelines recommend that pneumonia should be treated in a hospital with injectable antibiotics. With the new study there are indicators that pneumonia can be treated just as effectively at home with oral antibiotics.

There are few organizations in the United States that elicit a stronger emotional response than the American Civil Liberties Union, whose stated goal is “to defend and preserve the individual rights and liberties guaranteed to every person in this country by the Constitution and laws of the United States”. Those people include gays, Nazis, women seeking abortion, gun owners, SPAM mailers and drug users. People who are often not popular with various segments of the public. The ACLU’s philosophy is not that it agrees or disagrees with any of these people and the choices that they make, but that they have personal liberties that must not be trampled upon.

In Wikinews reporter David Shankbone’s interview with the President of the ACLU, Nadine Strossen, he wanted to cover some basic ground on the ACLU’s beliefs. Perhaps the area where they are most misunderstood or have their beliefs most misrepresented is their feelings about religion in the public sphere. The ACLU categorically does not want to see religion disappear from schools or in the public forum; but they do not want to see government advocacy of any particular religion. Gay Talese wants to go to Iraq. “It so happens there is someone that’s working on such a thing right now for me,” the 75-year-old legendary journalist and author told David Shankbone. “Even if I was on Al-Jazeera with a gun to my head, I wouldn’t be pleading with those bastards! I’d say, ‘Go ahead. Make my day.'”

Few reporters will ever reach the stature of Talese. His 1966 profile of Frank Sinatra, Frank Sinatra Has a Cold, was not only cited by The Economist as the greatest profile of Sinatra ever written, but is considered the greatest of any celebrity profile ever written. In the 70th anniversary issue of Esquire in October 2003, the editors declared the piece the “Best Story Esquire Ever Published.”

Talese helped create and define a new style of literary reporting called New Journalism. Talese himself told National Public Radio he rejects this label (“The term new journalism became very fashionable on college campuses in the 1970s and some of its practitioners tended to be a little loose with the facts. And that’s where I wanted to part company.”)

He is not bothered by the Bancrofts selling The Wall Street Journal—”It’s not like we should lament the passing of some noble dynasty!”—to Rupert Murdoch, but he is bothered by how the press supported and sold the Iraq War to the American people. “The press in Washington got us into this war as much as the people that are controlling it,” said Talese. “They took information that was second-hand information, and they went along with it.” He wants to see the Washington press corp disbanded and sent around the country to get back in touch with the people it covers; that the press should not be so focused on–and in bed with–the federal government.

Augusten Burroughs once said that writers are experience junkies, and Talese fits the bill. Talese–who has been married to Nan Talese (she edited James Frey‘s Million Little Piece) for fifty years–can be found at baseball games in Cuba or the gay bars of Beijing, wanting to see humanity in all its experience.

What Causes Spondylolisthesis And What Is It, Anyway?}

Hearing spondylolisthesis causes some individuals to wonder whether the speaker is using a foreign language. While it may have originally derived from the Greek words for vertebrae and dislocation, the word spondylolisthesis is, in fact, part of the English language. Understanding how to pronounce or spell spondylolisthesis is one thing, but comprehending what it is and how it is caused is another matter altogether. So what exactly is this odd-sounding condition and what causes it, anyway?

Spondylo-What?

Though its name makes the condition seem highly complex and difficult to comprehend, spondylolisthesis is actually quite simple to understand one you know a little bit about the spinal anatomy. The main components of the spine include:

Spinal cord The so-called information superhighway, the spinal cord is a large bundle of nerves that delivers sensory and motor signals from the brain to the rest of the body.

Spinal nerves These tiny branches stem from the spinal cord and travel throughout the body, providing feeling and mobilization for every part of our bodies, from our heads to our toes.

Vertebrae Bone structures that house and protect the spinal cord. Most people have a total of 33 vertebrae; nine are fused together to form the coccyx and the sacrum while the 24 individual vertebrae in the neck and back are articulated with each other.

Facet joints These synovial joints are tasked with connecting articulating, adjacent vertebrae.

Intervertebral discs Positioned between articulating vertebrae, these saucer-shaped pads absorb the impact that is placed on the neck and back when the spine facilitates movement and supports weight.

Now that youre aware of the various components of the spine and the tasks they perform, its time to explain what spondylolisthesis is and how it develops. Simply stated, spondylolisthesis is vertebral displacement that occurs when one vertebra shifts forward and over the vertebra beneath it.

When spondylolisthesis is diagnosed, the physician assigns a grade to the condition that identifies the extent to which the affected vertebra has shifted out of place:

Grade 1 0 to 25 percent slippage

Grade 2 25 to 50 percent slippage

Grade 3 50 to 75 percent slippage

Grade 4 75 to 100 percent slippage

Grade 5 the vertebra is completely displaced and is no longer positioned above the vertebra directly below it

What Does Spondylolisthesis Feel Like?

Interestingly, spondylolisthesis causes no symptoms in many patients. This has to do with the fact that neural compression is what often causes symptoms, not the actual displacement of a vertebra. Especially in cases of low-grade spondylolisthesis, the spinal nerves and the spinal cord are unaffected and, therefore, the patient remains unaware that they are even afflicted with the condition. Higher grades, however, can and typically do lead to pinched nerves or even compression of the spinal cord itself. This causes patients to experience radiating symptoms of pain, numbness, tingling, and muscle weakness. When spondylolisthesis occurs in the lower back (the portion of the spine most susceptible to vertebral displacement), patients may have difficulty walking and may appear to have a protruding abdomen.

What Causes Spondylolisthesis?

Believe it or not, the primary cause of spondylolisthesis and other degenerative spine conditions is simply the natural aging process. After supporting the weight of the upper body and facilitating every bend, twist, lift, and so forth, its no wonder that the neck and back tend to become sore and tired by about middle age. In fact, almost every portion of the spine is prone to degenerative changes, which occur slowly over time.

The facet joints are one of the primary victims of the natural aging process, as years of articulated movement can cause the cartilage that lines the joints to wear away. When the bones of the joints uncomfortably rub against one another, they can become inflamed, painful, and tender to the touch, and may be less able to perform their responsibilities, leading to loss of mobility. This is called facet disease, which is a type of arthritis and is also sometimes called facet syndrome or spinal arthritis.

The intervertebral discs also undergo several changes during the natural aging process. An intervertebral disc has a tough outer shell, called the annulus fibrosus, that can become weak and brittle with age. A discs inner material, called the nucleus pulposus, can lose water content and elasticity, preventing the disc from properly absorbing impact. Together, these changes can cause a herniated disc, which occurs when the annulus fibrosus ruptures and the nucleus pulposus seeps into the spinal canal, as well as a bulging disc, which occurs when the annulus fibrosus becomes too weak to properly support the nucleus pulposus, allowing it to shift out of place and balloon to one side.

So what do facet disease, herniated discs, and bulging discs have to do with spondylolisthesis? All of these degenerative spinal conditions lead to inadequate support of the vertebrae, which sometimes causes spondylolisthesis.

How is Spondylolisthesis Treated?

For the most part, a patients treatment regimen will depend on the extent to which the affected vertebra has slipped out of place. In the case of low-grade spondylolisthesis, most physicians advise their patients to initially utilize a conservative, nonsurgical treatment plan that is aimed at easing symptoms rather than on reversing vertebral slippage. This almost always entails the use of medication, primarily over-the-counter, nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or naproxen. Certain medical conditions, especially those that affect the gastrointestinal or cardiovascular system, prohibit patients from taking NSAIDs. Instead, these patients may be advised to take over-the-counter analgesics such as acetaminophen. If a patient is experiencing severe or debilitating pain, his or her physician may prescribe narcotic pain relievers or muscle relaxants.

In addition to medication, the patients treatment regimen may include one or more of the following:

Physical therapy

Cryotherapy

Thermotherapy

Lifestyle modifications (i.e. weight loss)

Transcutaneous electrical nerve stimulation (TENS)

Some patients with low-grade spondylolisthesis may receive little to no relief from nonsurgical treatments after several weeks or months. A minimally invasive procedure that serves to decompress an affected spinal nerve may provide relief, which can typically be achieved after a week or two of rehabilitation. These procedures are performed on an outpatient basis and utilize an endoscope to gain access to the spine through a very small incision.

High-grade spondylolisthesis causes more severe symptoms that cannot always be alleviated through the use of conservative techniques. Some of these patients may be advised to undergo spinal fusion surgery, which entails the installation of a bone graft and stabilizing hardware to reverse vertebral slippage and prevent it from reoccurring.

Neurofibromatosis (NF) is a genetic condition causing benign tumors (neurofibromas) to grow along certain types of nerves and, in addition, it can affect the development of bones or skin. There are several variants of the disease but type 1 and type 2 NF account for the vast majority of cases.

The disease manifestations can vary from very mild to severe. Major symptoms include growths on and under the skin; skin pigmentations called café au lait spots in type 1; acoustic nerve tumors and consequent hearing loss in type 2. Growths can affect nearly all parts of the body, and pressure on nearby structures can cause a wide variety of complications. There is a small risk that the tumors transform into malignant cancerous lesions.

NF is one of the most common single-gene human diseases; around 1 in 2,500-4,000 live births are affected by NF-1, whereas NF-2 occurs in about 1 in 50,000-120,000. Both type 1 and 2 are autosomal dominant conditions, meaning that only one copy of the mutated gene need be inherited to pass the disorder. A child of a parent with neurofibromatosis and an unaffected parent will have a 50% chance of inheriting the disorder. The gene responsible for NF-1 and possibly NF-2 is thought to function as a tumor suppressor gene.

In most cases of neurofibromatosis 1, patients can live normal and productive lives. In about 25-40% of patients there is an associated learning disability with or without ADHD. In some cases of neurofibromatosis 2, the damage to nearby vital structures, such as the cranial nerves and the brainstem, can be life-threatening. When tumors are causing pain or disfiguration, surgery is thus far the only proven beneficial treatment option.

Reggie Bibbs is a 43-year-old-man living in Houston, Texas. Mr Bibbs was born with a genetic disease called neurofibromatosis (NF), which causes him to develop tumors on his body (see infobox on the right). NF can be a subtle disease, but in Bibbs’ case it has left him with a disfigured face and deformed leg. But he is happy with the way he looks, and doesn’t want to change his appearance to please other people. He has launched a successful campaign entitled “Just Ask”, and that’s just what Wikinews did in a video-interview.

The interview was prepared by Wikinews reporter Michaël Laurent with the help of Bertalan Meskó (who has a popular genetics and web 2.0 blog). Their questions were sent to a close friend of Mr. Bibbs, Lou Congelio, who kindly conducted the interview.

200,000 litres of oil leaked into waters off the coast of Brisbane from the Pacific Adventurer when their fuel tanks were damaged in rough seas on Wednesday. The figure is about ten times higher than the original estimate of twenty thousand litres of oil. The devastating diesel oil spill has spread along 60 kilometres (37 miles) of the Queensland coast. In addition, 31 containers with 620 tonnes of ammonium nitrate fertiliser flew overboard during the violent storm.

Questions are being asked why the Hong Kong cargo ship was out in seas with nine meter waves caused by Cyclone Hamish, a Category 5 tropical cyclone, as well as why the fertiliser containers were not properly secured. One of the overboard containers ruptured the hull of the Pacific Adventurer, causing between 30 to 100 tonnes of oil to spew from the severely damaged ship.

If the ammonium nitrate mixes with the heavy oil, an explosion could occur. None of the containers have been recovered. Some of these may float, but it is believed that they may have sunk which then may cause algal blooms.

Disaster zones have been declared at Bribie and Moreton Islands, and along the Sunshine coast.

The vessel’s owner, Swire Shipping, reported that a second leak began on Friday, when the ship began listing after docking at Hamilton for repairs. “As full soundings of the vessel’s tanks were being taken at the port to determine how much oil had leaked from the vessel, a small quantity of fuel oil escaped from the Pacific Adventurer,” it stated. The ship was brought upright, and a recovery vessel was used to suck up the oil from the water. The leak produced a 500m-long oil slick down the Brisbane River. Booms were placed around this oil spill so that a skimmer could clean up the second spill.

Swire Shipping could face clean up costs of AU$100,000 a day as well as fines up to AU$1.5million (US$977,000; £703,000) if found guilty of environmental breaches or negligence.

Sunshine Coast beaches are slowly starting to be reopened. The beach of Mooloolaba was still closed following reports of burning sensations from swimmers. 12 beaches remain closed; however, 13 have been reopened.

Over 300 state government and council workers are using buckets, rakes and spades in the clean up effort. Sunshine Coast Mayor Bob Abbott says the majority will be gone by Sunday afternoon. The full environmental impact on wildlife is not yet known. One turtle and seven pelicans have been found covered in oil.

There are concerns that the drinking water of Moreton Island is at risk, as the island uses water from the underground water table near the oil spill site.

“Every bucketload of contaminated sand has to be removed from the island by barge, and each bucketload from a front-end loader weighs about one tonne. It’s just an impossible task,” said Mr Trevor Hassard of the Tangalooma Dolphin Education Centre.

The commercial fishing industry has suffered from the incident. Trawlers won’t resume operations until Sunday evening, and any catches will be tested for human consumption.

Born in London, England to Czechoslovakian parents, she immigrated to Canada in 1968, having a long career in human resources with the federal government. She is a volunteer for organizations including the Alberta Women’s Shelter, Big Sisters and Match International. She has previously run in campaigns three times provincially for the NDP, and once federally.

In Pakistan, the Daily Times reported that construction work on a sewerage project has started in Pindi.

It said that the construction work for the Rawalpindi Environment Improvement Project (REIP) aims to lay a new underground drainage and sewerage system in the area.

The project director of REIP, Aslam Sabzwari, said, according to the report, that improvement of sanitation and sewerage system had started in the eastern zone of the city, consisting of 15 union councils (UCs). The project includes improvement of the sanitation and sewerage systems, a sewage treatment plant, solid waste management, removal of slaughter-houses and construction of public toilets.

Sabzwari also said that 22 new tube wells would be installed, while 32 old ones would be repaired under the project, recalling that the existing sewerage systems in Satellite Town and Kahyabane-Sirsyed were laid in the 1950s and 1969s, respectively, and were the main cause of the increasing case of hepatitis in the city.

Asian Development Bank (ADB) and the Punjab government sponsor the REIP.
